What does readiness require?#
Five things together: a clear decision, the right expertise, enough understanding, the evidence to hand, and time to act. Missing any one of them produces an approval rather than a judgment, and the record cannot tell the difference between the two afterwards.

Why is "insert a human" the wrong question?#
Because a human gate where the person cannot really judge is not a safety mechanism. It satisfies a process requirement and adds delay, while leaving the risk exactly where it was. Worse, it transfers accountability to someone who was never in a position to exercise it.
Making people ready is what comprehension engineering is for, and time to decision-ready is how you tell whether it is working.
