Automate the check, or accept the risk explicitly. What is not an option is leaving a person there to produce an approval nobody treats as information — that buys the appearance of control at the price of real delay, and it degrades the meaning of every other approval in the system.

What should supervision follow?#
The consequence and reversibility of a change, not its size. A large, well-tested, cheaply reversible change needs less supervision than a one-line change to an irreversible path, and gates that key on diff size get this backwards almost every time.
A gate staffed by throughput police is system debt in its most visible form: a person standing where a check should be.
