What does an outcome check declare?#
Six things, all before the work starts: the signal, how far it should move, who it applies to, how long to watch, what must not get worse, and how to tell this change apart from others. Declaring them afterwards is how a disappointing result becomes a story about why the metric was wrong.

What happens when it fails?#
If something gets worse, the system rolls back automatically. What to do next stays a human decision — rollback is a safety action that should not wait for a meeting, while deciding whether to try again, change the approach or abandon the idea is a judgment about consequence.
This is the measurement end of the delivery loop, and it is what makes the loop a loop rather than a pipeline.
