What does the process record, and what does it miss?#
The process records oversight. Whether oversight happened is a separate question, and no audit trail distinguishes the two: an approval made after careful review and one made in four seconds look identical afterwards.

How does regulation frame it?#
The EU AI Act's Article 14 frames oversight in terms of what the overseer can understand and do — their ability to interpret the system's output and to intervene or interrupt it. That is a capability standard, not a staffing one, and a named approver who cannot meet it does not satisfy it.
The practical consequence is that oversight has to be engineered rather than assigned. Making someone decision-ready is the work; naming them in a process is not.
