Why must an approval name its scope?#
Because silence is not permission. An approval that does not say what it covers and which version it applies to will be read later as blanket consent, and the person who gave it will be accountable for work they never saw.
Naming the version is the cheap part and the load-bearing part. It is what makes an approval expire when the thing it approved changes.
